Welcome to the Admissions & Records Office. Use the links at the  bottom of the page or on the side menu to access information about many of our services.

Our staff can assist you with questions concerning your application, registration, fees, grades, transcripts, graduation, diplomas and certificates. If you would like to visit our office, please stop by Room A102 (off the Commons) during our regular business hours.

AB 540 Forms

  • AB 540 Non-Resident Tuition Waiver Form AB 540 Non-Resident Tuition Waiver Information (English Version) - see page 2 of the Tuition Waiver Form linked above
  • AB 540 Non-Resident Tuition Waiver Information (Spanish Version) Students who attended a California high school for at least three years may be eligible for a waiver of non-resident tuition. Please see the form for additional requirements.

 

Petition Forms

  • Challenge Exam Petition Form Please see the back cover of the Schedule of Classes for the deadline to register for Challenge Exams.
  • Petition for Academic Renewal or Exceptions to Academic Policies and Procedures Please speak with a counselor regarding the appropriate circumstances for using this petition. Additional information on academic renewal requirements is available in the college catalog.
  • Petition to Register (pre-grade posting) Courses completed with a grade of D, F or NC may be repeated once, but not for duplicate credit. When registration begins prior to the anticipated substandard grade being posted, the student may conditionally enroll in the course by providing documentation of the expected grade.
  • Petition to Repeat Course (Substandard Grade) Courses completed with a grade of D, F or NC may be repeated once, but not for duplicate credit. Additional repeats require an approved petition to repeat the course.
  • Student Petition In cases of extenuating circumstances beyond the control of the student (such as death of a family member; military call to active duty or extended hospitalization), a student may petition for an extension of the refund, drop and withdrawal deadlines. Approval of the petition is NOT automatic and documentation of the extenuating circumstances should be included.

 

Registration Forms

  • Conflict Waiver Form In general, a student may not register for two courses with overlapping times. On rare occasions involving small amounts of time, the student may be allowed to register upon approval of a Conflict Waiver.
  • Credit Card Authorization Form Most fees may be paid online by using WebReg. If you experience difficulties with online payment, you may pay your fees in person or use this form to submit your payment by fax. NOTE: Fees are due at the time of registration. To avoid being dropped for nonpayment, please check with Admissions & Records to ensure that your fax was received and that payment has been processed.
  • Late Registration Permit This form may be used by students to register late with instructor permission during the first two weeks of the class. Students must bring this form to Admissions & Records during business hours before the end of week 2 to officially enroll in the course.
  • Online Course Late Registration Permit This form may be used by students outside the Tahoe area to register late for online courses during the first two weeks of the quarter.

  • Age 19 and Under - Supplemental Residency Questionnaire Please use this form only if you are asked to complete it by the Admissions & Records staff.
  • Supplemental Verification of Identity Please use this form only if you are asked to complete it by the Admissions & Records staff.

 

Other Forms

  • General Release Form This form may be used by students to release to authorize the release of specified educational records to their parents, employers, or other individuals.
  • Request to Prevent Disclosure of Directory Information Student records are protected under the Family Educational Rights and Privacy Act (FERPA). Please see the college catalog for more information
